Ramsay Connect is a national provider of home and community-based healthcare and support. We work closely with Ramsay Health Care hospitals to support the transition of individuals from hospital to home, providing seamless and integrated quality healthcare in the home and community setting. Ramsay Connect understands healthcare extends beyond the hospital and works with individuals to ensure health and wellbeing goals are addressed after leaving hospital. The first days and weeks following a hospital stay can be difficult. Ramsay Connect assists through this period, supporting individuals to thrive in their home as quickly as possible. The healthcare professionals providing the program are experienced in community care and committed to optimising each person's recovery.
