HomeScript Pharmacy Services Information

HomeScript is a local, independently owned and operated full service pharmacy offering free delivery of prescription medications to patients' homes. Our patient centric approach utilizes the following tools: • Proprietary software developed exclusively for HomeScript that tracks patient medication utilization and prompts pharmacy staff to call a patient when their medications are running low • An unmatched level of customer service that includes monthly or as needed patient reminder calls and free delivery of medication • In appropriate situations, a home visit by a trained pharmacy technician to review hospital discharge medications and other factors which may lead to non-adherence or complications. Our goal is to provide medication adherence and prevent readmission to the hospital. As a result of the improvements in patient medication adherence driven by the HomeScript program, costs and complications for our patients have been reduced.
