PlanCare is a healthcare-at-home platform and service that enables our older generation to self-manage their care and support, giving them more choice and control and access to more services and providers at a lower cost than the traditional care model. PlanCare connects the over 65’s Healthcare-At-Home Ecosystem, from those requiring support, their family members and caregivers to the providers they use and the funding they need. Our innovative platform brings everyone together to reduce the stress of coordinating supports and provide clarity on supports, including fund and budget management, whilst monitoring the well-being of our users.
