Immedicare Information

Immedicare is an NHS digital clinical service providing care home residents with timely, expert support 24/7 in their home and community. Aligned with the NHS 10-year health plan and its three big shifts: hospital to community, analogue to digital, and treatment to prevention, Immedicare helps translate strategy into frontline impact. We enable timely clinical input, reduce avoidable hospital admissions, and strengthen integration between care homes, community teams, and neighbourhood health models. Care home teams can access secure video consultations with multidisciplinary NHS clinicians to support falls prevention, wound care, medicines safety, nutrition, hydration, and end-of-life care. Around 90% of residents remain safely in their home following a consultation, supporting proactive, integrated, person-centred care.
