Hospice of Northern Colorado Information

Hospice of Northern Colorado is a non-profit community based hospice providing quality end-of-life care since 1978. At Hospice of Northern Colorado we have trained professionals that can address the many needs of someone facing the end of life. We treat not only the physical, but the emotional and spiritual needs of our patients and their families by providing a family centered approach to health care. We provide the highest quality of care directly to the patient in home, in a nursing home or an assisted living facility. We are staffed with nurses, certified nurse’s aids, chaplains, social workers and a medical director, who work directly with your primary care physician to design a plan of care, specific to you and your family. We care for patients of any age, at the end stages of any disease. Hospice of Northern Colorado offers our patient's family with 13 month of bereavement services after the passing of their loved one. We provided grief education, groups and counseling to anyone in need of our services.
