CollabCare Information

CollabCare is aimed at changing the way we look at care coordination and delivery for patients. We create solutions for the healthcare market that focus on increasing access to health care services as well as enabling patients to engage in their own care while maintaining the crucial support structures provided by clinicians, carers and family members. We develop our case-specific pathways based on the care plan instructions developed by clinicians. Our role is to fill in the gaps in coordination by making the care plan easy to understand and follow while also supporting the patient through their care journey.
