Swanton Care & Community is a value based organisation, committed to personalisation and putting the people we support in control of their own decisions and choices. We focus on enablement and maximising independence to work with people who use our support be co-producers of their own goals and outcomes. Founded in 2006 with a large number of its services having existed for many years prior, Swanton operates across three regions - Scotland, the North East and Hull; South Wales and West Midlands; and East Anglia. We provide a range of different support solutions including registered care homes, care homes with nursing, community based supported living and domiciliary care across three operational areas. We support people in a number of services and are able to demonstrate how, with the right support and staff we have been able to increase people’s independence, decreasing their reliance on staff (and therefore reducing costs) and increasing quality of life.

Potens has been providing a range of specialist support services in the community for over 35 years. We support adults and children with complex needs including learning disabilities, mental ill health, autism, behaviors that challenge, profound and multiple disabilities, forensic provision, and Korsakoffs syndrome (alcohol related brain injury) through registered support and independent living, domiciliary care, residential care, respite, day opportunity and skills services and independent schools throughout England, Wales and Northern Ireland.
