Since 2008, the Zuellig Family Foundation (ZFF) has worked with rural municipalities across the country, using a Health Change Model that serves as a framework for partnerships with local health leaders to achieve better health outcomes, especially among the poor. The Model has succeeded in helping ZFF partner municipalities improve health indicators by transforming the local health system. In particular, responsive health leadership, coupled with new institutional arrangements and innovations in health programs and services, have reduced MMR and IMR in ZFF cohort municipalities in as little as two years. Anchored on the premise that health leadership is key to public healthcare reforms, the Foundation’s Health Change Model aims to help local chief executives, municipal health officers, and other health professionals, as well as community leaders, improve health governance and make local health systems equitable, responsive and sustainable. The Model promotes institutionalized and targeted health interventions designed to impact on the health conditions of the poor, which are measured against the country’s commitments to achieve the Millennium Development Goals (MDGs) on health. Vision: "To be a catalyst for the achievement of better health outcomes for the poor by strengthening leadership and governance, with primary focus on rural communities and secondarily, urban poor communities, in the Philippines" Mission: "To enhance the quality of life of the Filipino by focusing on the achievement of targets in the country's Sustainable Development Goals for health, in partnership with government and other stakeholders'' Goals: 1. All trained local health leaders have improved bridging leadership competencies 2.
