ZONH, Zorg Optimalisatie Noord-Holland, ondersteunt eerstelijnszorgverleners in het verbeteren van de samenhang en het leggen van verbindingen rond zorg en welzijn van burgers in Noord-Holland. ZONH concentreert zich op regionale en lokale ondersteuningsvragen. Daarnaast stimuleert ZONH samenwerking met andere regionale partners, zoals provincie, gemeenten, woningcorporaties en zorg- en welzijnsinstellingen. ZONH is de Regionale Ondersteunings Structuur in de regio Noord-Holland m.u.v. Amsterdam en 't Gooi Regional support organisation for primary health care, including GP's and fysiotherapists. Main goal is to support coherence and cooperation wth and within the primary health care in the northwest region of the Netherlands.
