The Pain Care Center strives to provide the highest quality in individualized rehabilitation services through an interdisciplinary twenty-one day program designed to treat patients with multiple, complex problems. The scope of outpatient treatment and services concentrate on the individual as a whole: including the mind, body and spirit. An interdisciplinary treatment team will provide an individualized plan of care developed with the patient. The patient is considered to be a member of the treatment team and regularly participates through feedback in team conference. Both comprehensive and modified treatment plans and members of the team are designed depending upon the individual and special needs of each patient. Assessment of the patient's treatment plan and progress towards goals is an ongoing process. Weekly reassessments are done by The Pain Care Center physician, Physical Therapist, Psychologist and Program Coordinator and are discussed with the patient in team conference.
