The Visiting Nurse Association of Porter County has provided care for more than 40 years to individuals who need health care at home in Porter County and portions of Lake, Jasper, LaPorte and Stark couinties. The VNA provides a wide range of comprehensive programs and services including skilled nursing care, chronic disease management, end of life care. Services are provided in the patient's place of residence or at the Arthur B. and Ethel V. Horton Hospice Center. With Telehealth communication technology, clinical data on patient's health is collected with a monitor placed in the patient's home and transmitted to VNA healthcare providers. In addition, we provide Meals On Wheels, Companion Homemaker, Lifeline and volunteers for our Hospice patients. The VNA Phoenix Center is a unique program for children, teens and young adults grieving the death of a significant person in their life. Peer support in a safe, healing environment helps normalize the way these young people are feeling.
