Founded in 2026, True LTC is an Enhanced Track Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO) that partners with primary care providers serving traditional Medicare beneficiaries in long-term care and assisted living settings. True LTC was established by the team that created the first MSSP ACO dedicated to long-term care providers and beneficiaries more than a decade ago. Building on that experience, True LTC is intentionally designed to return a greater share of savings to the providers delivering high-quality, coordinated care. Our mission is to create an ACO that truly works for providers. Guided by transparency, accountability, and trust, we provide clear insights into performance throughout the year and strive to eliminate surprises at settlement. By aligning incentives and supporting provider success, we help practices improve outcomes while maximizing their shared savings opportunities.
