St. Francis Pathways is dedicated to using a customized care team approach to pain and symptom control; social, emotional and spiritual needs; therapy needs; all in support of your patient. This includes establishing goals for your patients care and coordination of on-going care. Emotional support: the St. Francis Pathways approach focuses on the entire patient, not just the illness. The care team members address any social, psychological, emotional or spiritual needs your patient may have. Family/caregiver support: our St. Francis care team not only works in support of the patient, but also focuses on easing some of the strain and burden caregivers have so they can focus on just being with and enjoying the time they have with their loved one.
