Founded in 1998 by Ann Marie Neufelder, a pioneer in software failure modes analysis with over 35 years in the field and author of the definitive book on software FMEA, we've built the largest known database of real-world software failure data — nearly one million analyzed failure reports spanning defense, space, automotive, medical, and energy systems. That data is what makes our models accurate instead of theoretical. We literally wrote the standard: our team developed the Common Defect Enumeration (CDE) hosted on the DoD R&M Community of Practice website, and co-authored the Reliable Software Statement of Work with the US Army — the SOW clause set now flowing down through nearly every DoW software-intensive contract, from primes to subsystem vendors. That SOW requires nine specific reliability engineering tasks: a Reliable Software Program Plan, software failures in FRACAS, software in the system reliability model, allocation, prediction, evaluation, software FMEA, risk assessment, and reliability testing. Contractors typically have just 90 days post-award to define how they'll execute all nine — and most don't have the staff or tools to do it. That's where we come in.
