Self-Direct Inc is a NYS Department of Health (DOH) Licensed Home Care Agency, established in 2003 with offices located in Baldwinsville and Utica to provide professional home care services and home medical equipment and supplies. We provide services under the NYS DOH Traumatic Brain Injury and Nursing Home Transition and Diversion waiver programs as well as working closely with NYS managed long-term care (MLTC) programs, insurance companies and private pay clients.
