Rural America is losing access to healthcare. Over 730 hospitals are at financial risk, provider shortages are accelerating, and the patients who need the most help live the farthest from it. RHT Pathways exists to change that. The CarePath solution suite from RHT Pathways doesn't ask rural communities to find more clinicians; it makes the clinicians they already have dramatically more effective. Built on a Distributed Care Model, CarePath shifts healthcare delivery from role-based to skills-based, enabling every nurse, paramedic, and physician to operate at the top of their capability because the right information reaches the right person at the right time. At the center of the CarePath solution is the AkeLex Adaptive Knowledge Engine, an ontological AI expert system (not a large language model) that monitors thousands of patients simultaneously using deterministic, evidence-linked clinical reasoning. Remote Patient Monitoring, digital care journals, medication adherence tools, and a 24/7 Virtual Nurse triage team work together to catch problems early and route intelligent escalation notifications to the appropriate level of care, whether that is Community-Based Paramedicine, Urgent Care, EMS, or a scheduled follow-up. The result: 3-5x more patients per clinician without sacrificing safety. Safety is not an afterthought. Our architecture incorporates MIT's STAMP/STPA systems engineering framework to rebuild the control structures rural healthcare has lost: timely feedback loops, consistent information flow, and shared mental models across care settings. Closed-loop control means hazards are identified early, and unsafe care actions are prevented before they reach the patient. We partner with states, health systems, and community organizations to deploy CarePath where it is needed most.
