When an agent at 3am decides which patient deterioration warrants a clinician, or which fleet asset gets pulled from rotation, or which drone to maneuver on a battlefield, two questions decide whether that decision survives review: did the agent act on what was actually true, and can you reconstruct exactly why it acted? In complex environments, autonomous agents need a governed operating picture before action and a durable evidence trail afterward. Without both, intelligence becomes an unreviewable collage of APIs, documents, model outputs, and stale context. The central infrastructure problem for agentic software is therefore two-sided: give the agent a governed, comprehensive view of the world before it acts, and preserve the evidence needed to reconstruct the decision after it acts.
