Probari is an Indiana-certified Women's Business Enterprise (WBE) dedicated to becoming the standard for nursing facility care. The program was born out of a $30.3 million CMS grant, through which our team demonstrated consistent, proven outcomes — improving care transitions and reducing avoidable hospital transfers by one-third. Research shows that two-thirds of 30-day hospital readmissions are avoidable. Probari bridges this gap through its Care Transitions Program, pairing registered nurses — who bring a combined 200 years of nursing home and specialized geriatric care experience — with systematic clinical workflows. Acting as an additional set of clinical eyes, Probari currently supports more than 140 facilities across Indiana and Illinois, reducing hospital readmissions by 25% and generating average savings of $16,000 per prevented readmission. Probari's care model is the result of a $30 million demonstration project called OPTIMISTIC, aimed to uncover the challenges facing the post-acute and long term care facility industry and create systematic processes to revolutionize nursing home care.
