Primary Care Connect Ltd Information

It is a not-for-profit organisation that is designed to bring together local understanding and knowledge of Liverpool general practice and the strength and capacity of a large NHS community trust organisation. Primary Care Connect has been established to provide professional well-run GP services within Liverpool by the people who know the service better than most - Liverpool GPs themselves. All parties within Primary Care Connect will work together with the Liverpool Clinical Commissioning Group and NHS England to improve the quality of the care provided to local patients. From 1st April 2017, Primary Care Connect will be managing the GP practices listed below:West Speke Health CentreGarston Family Health CentreNetherley Health CentreAnfield HealthPark View Medical CentreEverton Road Health CentrePrimary Care Connect will be managing the above GP practices in Liverpool following selection by the Liverpool Clinical Commissioning Group. They are known as Alternative Provider Medical Services (APMS) practices. That means they are run by an organisation that are seen as best meeting local needs but are part of the NHS and represent the core NHS values. All the Primary Care Connect GP practices will maintain their independence but work collaboratively for the benefit of patients. Patients will be at the heart of everything we do. By working as a team of practices, patients and staff will benefit from Primary Care Connect's ability to identify and implement innovative ways of working across the practices. We will also be working closely to support and integrate community health and social care within a general practice setting so that NHS services are better joined up for patients. Mentor Practices
