Next we develop and discuss a customized plan of care. Based on the assessment, a plan of care is designed with the family...this includes day-to-day routines, long-term goals and ambitions. Then we pair you with a properly matched custom caregiver or team. Taking personalities, needs, and qualifications into consideration, we properly pair a caregiver, or team of caregivers, to fulfill your family care plan. Finally, we continuously monitor our progress and care plans, and update levels of care as goals are reached or new challenges arise.
