Unity Care is a 501(c)3) organization headquartered in San Jose, CA, dedicated to breaking the cycle of poverty and homelessness for young adults aging out of foster care. Serving over 500 young adults throughout Alameda, Monterey, Placer, San Francisco, and Santa Clara Counties in Northern California, Unity Care’s 5 Pillars of Support Program Model helps reverse negative trends for young adults aging out of foster care. Through a trauma-informed and proactive support model, we help these youth build confidence, feel hopeful about their futures, and achieve career success and financial self-sufficiency as they transition into adulthood.
National Harm Reduction Coalition is a national advocacy and capacity-building organization that promotes the health and dignity of individuals and communities impacted by drug use. NHRC advances policies and programs that help people address the adverse effects of drug use including overdose, HIV, hepatitis C, addiction, and incarceration. We recognize that the structures of social inequality impact the lives and options of affected communities differently, and work to uphold every individual's right to health and well-being, as well as in their competence to protect themselves, their loved ones, and their communities.
