Prescription drug costs are increasingly concentrated in a small group of high-cost specialty therapies. For many employer health plans and union funds, a handful of medications can drive a disproportionate share of pharmacy spend and stop-loss volatility. PlaidRx was built to address that challenge. It’s a voluntary, member opt-in pharmacy cost optimization overlay that works alongside a plan’s existing PBM. The program focuses on select high-cost specialty maintenance medications and delivers savings upfront at the point of dispensing rather than relying on delayed rebate reconciliation. Participation is entirely voluntary, and members who do not opt in continue using their pharmacy benefit exactly as they do today. For participating plans, savings typically range from 25–45 percent on certain therapies depending on drug mix and plan structure. Clinical areas of focus include autoimmune disorders, inflammatory bowel disease, oncology, multiple sclerosis, HIV and infectious disease, immunology, respiratory conditions, neurological disorders, and hematologic disorders.
