Cordon Health builds Outbreak Watch, a bidirectional platform connecting congregate settings – skilled nursing facilities (SNFs), long-term care and assisted living, and correctional facilities – with the local health jurisdictions (LHJs) responsible for containing the outbreaks that start there. The tools that exist today are incomplete and unconnected. A facility can recognize a sick resident; what it cannot reliably do is accumulate cases against an outbreak threshold, apply the case definitions that state reporting law requires, and know when the threshold has been crossed. When it does report, the report goes out by phone, fax or email, and guidance comes back by phone and lives in nobody's record. The facility and the LHJ never work the same outbreak from the same record. Outbreak Watch closes that loop: one submission, routed to every channel that accepts one, with a shared, timestamped record on both sides of the boundary. It does not add a platform; it retires a stack of ad hoc tools and replaces them with one. Cordon was founded by people who managed outbreak response at California LHJs and inside California facilities during COVID, and who spent that time working around tools never designed for the job.
