The plan of care is based upon problems, needs, and goals identified by your physician's orders for medication, treatments, safety, nutrition, functional limitations, allergies, permitted activities, mental status, and prognosis. 123 PHHC will utilize information gathered in coordination with your care team during our comprehensive patient assessments care planning process, which involves the identification of patient goals, and nursing interventions that will resolve or alleviate your unique problems and/or needs. We encourage you, your caregiver or your designee to participate in the development of the plan of care and any revisions of the plan.
