Peninsula HealthCare Connection Information

Peninsula HealthCare Connection (PHC) is a not-for-profit organization with a focused mission to serve the needs of the homeless and the at-risk of becoming homeless. PHC services the north peninsula of the San Francisco Bay area. Medical, psychiatric and intensive case management is provided within a multi-service building complex with three other entities; Downtown Streets Team, InnVision Shelter Network, and the Housing Authority. The organizations operate cooperatively in partnership to deliver health care, social services and housing. PHC provides services through a collage of community agencies consisting of staff, trainees and volunteers including the Palo Alto Medical Foundation and Stanford University as well as the generosity of many.
