Welcome to Oak Park Healthcare center located at 625 North Harlem ave Oak Park IL 60302. The Village of Oak Park is well known to it’s neighbors as the “Tree City”. As such, Oak Park Healthcare center is nestled in a residential area with many trees, outdoor recreational area, and safe walkways for our residents to enjoy. Oak Park Healthcare Center is a 202 bed skilled nursing facility providing a variety of services ranging from 24-hour skilled nursing care, individualized therapies, Alzheimer’s/Dementia Care, Hospice Care, Respite Care. Oak Park Healthcare has Three floors of resident caring. The first floor is designed to meet the needs of residents with medical conditions, including our hospital step-down Medicare wing. The second floor is primarily designed to meet the needs of residents with Alzheimer's, Dementias, and other psychiatric diagnoses who have skilled needs . The third floor named "The Village" is all memory care with no skilled needs. We are licensed by the State of Illinois and are approved for Medicare, Medicaid, Medicaid Pending and many private insurances.
