From just one easy appointment, health plan members receive a detailed picture of their health, with the time to get their questions answered. After the visit, Signify Health shares a comprehensive report with the member, their primary care provider and their health plan. Our proactive approach enables members, health plans and health care providers to act on the information we provide efficiently and effectively. Ensuring a more connected, effective care experience for all.

Every person in the Netherlands is entitled to health care offered in the basic health insurance package. We all contribute to this through our health insurance. The National Health Care Institute determines and advises on which types of health care are included in the basic care package and which are not.

Pharos is het landelijk expertisecentrum dat zich bezighoudt met vraagstukken rondom gezondheidsverschillen en toegankelijkheid van de zorg. Pharos werkt gezondheidszorgbreed. We hebben een breed en gevarieerd netwerk met opdrachtgevers, veldpartijen, professionals, patiënten- en cliëntenorganisaties, migranten- en vluchtelingenorganisaties en zelforganisaties. Als specialistisch kenniscentrum maakt Pharos graag gebruik van de expertise van kenniscentra en universiteiten. We werken regelmatig samen met hen in programma’s en projecten.
