My Choice Family Care is a nonprofit managed care organization that matches frail seniors and people with physical and intellectual disabilities to the health care and long-term care supports and services they need. My Choice Family Care is committed to our members independence! A leader in providing the Family Care program in Wisconsin, My Choice Family Care is a robust and thriving operation celebrating 17 years of successfully serving the community. Since 2000, we have provided the Family Care program to over 27,300 people in Wisconsin. Today, we serve over 8,400 Members.
