When you choose Morning Star Home Health Care, Inc. the following State required documents will be provided: List and Scope of Services Client Service Plan Certificate of Liability and Worker's Comp Insurance Daily Service Log Medication Chart Home Care Service Chart We will provide you with a contract outlining our responsibilities to you , our hourly rate and our billing procedure. Our contract is a "PROMISE TO PAY" for only the hours of service we provide. We are available 24 hours a day, 7 days a week.
