We Provide These Following Services Case Management Services Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s needs. Our case managers work with clients to advocate for their educational, legal, housing, employment, healthcare, and mental health needs. We help clients and their families access resources and encourage them toward independence. Outpatient Counseling Outpatient Counseling is designed to serve adults with mental health challenges, substance abuse, or other co-occurring disorders as well as children with mental and behavioral health needs. The program empowers individuals to build skills needed to begin the recovery process. Services focus on recovery and care coordination, with an emphasis on evidence-based practices and client-directed goals and outcomes. Rehabilitative and Community Support Services Treatment Services for Children with Cognitive Impairments and Functional Limitations are medically necessary services for children and youth under the age of 21. Treatment services are designed to retain or improve functional abilities that have been negatively impacted by the effects of cognitive or functional impairment. Our service focuses on behavior modification and management, social development, and acquisition and retention of developmentally appropriate skills.
