MDC PCN Information

Mottingham, Downham and Chislehurst Primary Care Network (PCN) comprises of 2 Practices in Bromley. The practices have a combined geographic area made up of around 36,000 patients. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. We strive to improve quality of care and wellbeing of our patients across the network of practices. Our PCN works in collaboration with various health and social care providers in Bromley to enhance and personalise patient care within our network.
