The mission of the Institute for Family Health is to improve access to high quality, patient-centered primary health care targeted to the needs of medically underserved communities.
As a federally qualified community health center network, we meet national standards for affordable, accessible, and comprehensive health care services.
In addition to operating health centers, we use our expertise and resources to address racial and ethnic disparities in health, advance the use of health information technology, and find innovative ways to improve care for diabetes, depression, women’s health, HIV and other priorities in the communities we serve.
The Institute for Family Health is committed to training the next generation of health professionals to provide high quality, family-oriented care to people in underserved communities. The MENTOR Network, now becoming Sevita, is a leading provider of home and community-based health care, inspired by our core values of respect, integrity, inclusion, and growth. We’ve been a pioneer since our founding in 1967. Today, our 40,000 team members proudly serve more than 50,000 individuals across the United States, providing the care and support they need to thrive.
Our continuum of care addresses the clinical, behavioral, and social drivers of health and well-being. We deliver individualized, integrated care to individuals at home and in the community. We enable independence, enhance quality of life, lower health care costs, and improve outcomes regardless of the physical, intellectual, or behavioral challenges people face.
Whether we are providing 24/7 nursing support to someone recovering from brain injury, ensuring someone with a developmental disability will thrive living independently for the first time, or providing ABA therapy to a young child in one of our schools, we are committed to advancing every person’s right to live well.
We are constantly designing new solutions to address the challenges of a complex, fragmented health care system. Partnership fuels our innovation, whether partnering with payors to integrate solutions or working with state agencies to address the growing demand of individuals to be served in the community. Our team’s innovative, person-first, and tech-enabled approach allows us to broaden and deepen how we serve each individual.
