Heartland Community Health Center is a Federally Qualified Health Center that operates an integrated primary care practice that exists to serve all community members regardless of income or insurance status in Douglas County and the surrounding area. Heartland provides primary care services, behavioral health and dental as a NCQA recognized Patient Centered Medical Home. Additionally, Heartland offers the following health and wraparound services for all patients: Medication Assisted Treatment for Substance Use Disorder, Peer Support Services, Ryan White Medical Case Management, Compass – Insurance Eligibility Assistance, Nutrition Education Classes, Food Pantry, Spanish Language medical interpretation and Care Coordination. Heartland also offers discounted medication through its inhouse repository and 340B Pharmacy program. At Sherbourne, a Family Health Team can comprise physicians, nurses, health promoters, mental health counsellors, outreach workers, dietitians and client resource workers, all tailored to the needs of the client.
