In September 1996, Home-Health Care Partners received its license to provide private home care services and now serves nine counties in upstate New York (Fulton, Hamilton, Herkimer, Montgomery, Saratoga, Schenectady, Schoharie, Warren and Washington counties). We are jointly sponsored by Nathan Littauer Hospital and St. Mary's Healthcare, and we are affiliated with our sister agency Community Health Center. As a not-for-profit, experienced home care agency with many talents, we are excited to enter our 20th year of service and be available when you need help the most. Our mission is to provide the very best care possible to people at home and in the community, and we do this by staying true to our values of integrity, compassion, safety, and dignity. Home-Health Care Partners' administrators are:Karen Clark, Ph.D., Executive DirectorKathleen Bonville, RN, Director of Patient ServicesJeannie Moller, Executive AssistantWe have an exceptionally experienced group of clinical staff that includes registered nurses, licensed practical nurses, certified home health aides, personal care aides, companions, and housekeepers. We provide medical services, end-of-life care, and non-medical services anywhere from a few hours to 24 hours a day for brief or extended periods when needed. Our staff are very good at meeting people's needs. All staff meet NYS requirements, are carefully screened prior to employment, are supervised in the field, and are provided with a strong education program during their employment with us. Home-Health Care Partners also is proud to serve as a Home Health Aide Training Program site in accordance with NYS Department of Health guidelines.
