Home and Community Care Support Services Central Information

The Central Local Health Integration Network (LHIN) is one of 14 LHINs established by the provincial government through the Local Health System Integration Act (LHSIA), 2006, to plan, coordinate, integrate, fund and deliver health services at the local level. The Central LHIN works with local health service providers and communities to design real-life solutions that improve access to care, better coordinate services and improve people’s experience with the health care system. As part of its expanded role, the Central LHIN provides home and community care services to over 89,000 patients each year. We work locally to connect people of all ages to the care they need at home in the community, ensuring people have the supports they need to return home from hospital faster, heal at home and to live at home longer, independently and with dignity. The services we provide include nursing, personal support and therapy services, as well as specialized services such as rapid response nursing, palliative care and child and family services. We also help people explore other care options such as adult day programs, retirement homes, short and long-term care and supportive housing. We serve more than 1.9 million people who reside in the highly diverse communities of South Simcoe, York Region, North York and areas of northern Toronto.
