HolistiCare is a care management organization that partners with home care agencies and supports individuals and families navigating complex health needs. Our mission is to set new standards in home care by integrating physical, behavioral, and social health through a proactive, whole-person approach. At HolistiCare, we believe care is about more than checklists. It's about integrating clinical, behavioral, environmental, and community supports to address the full picture of an individual's health. We provide more in-person visits than typical providers, bringing skilled nursing, behavioral health support, chronic care management, medication management, therapies (PT/OT/speech), and wellness visits directly into the home. Every service is fully reimbursed, meaning no cost to our partner agencies or the clients they serve. Our care managers don't just react to problems. They anticipate them. Through proactive check-ins, real-time data, and structured escalation workflows, we identify risks early, before a manageable issue becomes a hospital visit. The result: fewer avoidable readmissions, stronger outcomes, and greater peace of mind for families and providers alike. We serve two audiences: • Home Care Agencies- We fill clinical gaps, reduce administrative burden, and serve as a clinical extension of your team and a partner in value-based care. • Individuals & Families- We guide you through managing chronic conditions, coordinating providers, and accessing eligible services, so nothing falls through the cracks.
