The HealthFlow platform will provide a patient-centric care coordination capability that enables seamless and secure exchange of the right patient and care delivery information to the right provider, at the right time, regardless of source system or care treatment modality (in person or virtual). Leveraging AI and clinical process automation, we reduce provider cognitive load and administration overhead, allowing them to focus upon patient care. In turn, our platform enables patients to actively engage and manage their health through SDOH data integration, contextual reminders, enhanced health data access, healthcare team management, and a direct communication path to their provider team.
