HCRpath is dedicated to improving healthcare for all seniors. We believe this starts with making sure Medicare-covered seniors have access to preventative benefits that are included in their health plan. This access begins with the Annual Wellness Visit. We are committed to shedding light on the systemic problem of seniors—particularly those in minority communities—not getting access to the preventative care to which they are entitled. HCRpath simplifies the Medicare Annual Wellness Visit and enables providers to deliver a better patient care experience helping integrate wellness visits into their service offerings easily and efficiently. HCRpath combines health and wellness screenings with a cross-reference of the patient’s Medicare-covered preventative appointment history.
