HaloCare provides comprehensive disease management services to funder beneficiaries and employees living with a number of chronic conditions. HaloCare’s accreditation for the management of chronic conditions includes: • Cardiovascular • Diabetes • HIV/AIDS • Hypertension • Hyperlipidemia • Maternity • Mental health • Oncology patient care • Respiratory Core programme objectives: • Increase medicine and therapy adherence • Contain risk and cost • Partner with treating physicians and provider networks • Pro-actively identify high-risk patients • Actively monitor condition severity and target activities accordingly • Consider enrolled patients’ associated co-morbidities • Reduce unnecessary hospital admissions • Educate patients • Ensure patient compliance • Enhance quality of life We currently offer services in Namibia, Botswana and South Africa.
