Flathead Choice Home Health Agency Information

It is the mission of the Flathead County Home Health Agency to realize our vision through the delivery of caring and professional nursing, therapy, aide, and social in-home services to the residents of our community as demonstrated by: Service, Proactivity, Integration, Collaboration, and Commitment. Flathead County Home Health is managed by Paradigm Management PC, which was formed by Paul Conrad and Karol Conrad in 2003. Paul and Karol have extensive experience in management, Paul having owned and operated therapy clinics in Denver, Colorado as well as their own clinical backgrounds as physical therapists. Paul and Karol spent several years running the Physical Therapy department for Flathead County Home Health after moving to the Flathead Valley. When the contract for management of Home Health became available, they were excited by the opportunity to lead the agency. Flathead County Home Health operates under the guidelines of the Flathead City-County Health Department but is a self-sustaining department under the guidance of Paradigm Management PC. As we are a smaller agency in a rural community, it gives us the ability to do more individualized and personalized care to our patients. The staff is like a family, and always willing to go that extra mile to help their patients!
