First Mile Care is a Silicon Valley-based preventative chronic care company developing affordable, scalable, and sustainable solutions to reverse health conditions like prediabetes. We deliver an affordable, scalable, and sustainable solution that combats prediabetes in the community, providing people with the human support and tools necessary to live better, healthier lives. We do this through our coaches – local heroes trained to help fellow community members make the right decisions on a daily basis so they can take control of their health. First Mile Care has achieved the Certificate of CDC Full Plus Recognition for its Diabetes Prevention Program (DPP). This year-long program is available to participating physicians’ qualified patients and is covered by Medicare.
