evolutio Care Innovations Ltd (evolutio) offer a cost effective referral management gateway (‘eRefer’) with central administrative support (‘eCare’) and managed clinical networks for patients with common ophthalmology conditions. Through a sustainable and integrated model to enhance the quality of care for patients and overall patient experience, evolutio drive effectiveness and efficiency throughout the patient journey by reducing fragmentation of care, enhancing the integration between primary, community and secondary care and reducing the number of avoidable follow up appointments in hospital. eRefer is a unique IT solution that allows community care referrals to be received, triaged and managed appropriately through a shared electronic medical record that is not reliant upon implementation of the N3 network. evolutio supports both national and strategic objectives of providing care closer to home, delivering evidence based pathways and supporting patient choice whilst supporting CCGs’ QIPP Plans through innovative working and provision of cost-effective alternatives to secondary care. The purpose of this service is to reduce avoidable referrals to the hospital eye service (HES) and enable integrated community care whilst leveraging the associated savings. Through the provision of an integrated shared electronic referral record, unique to evolutio and supported by a referral management service, significant improvements will be seen in clinical standardisation, activity, data and quality of care. In turn, this will release additional capacity for stretched hospital clinics, reduce patient anxiety as treated in a more convenient setting and result in financial savings through the provision of clinically appropriate pathways within a community setting at a lower tariff.
