EPOCA is a medical remote monitoring solution designed for older adults living with multiple chronic conditions, at home and in care settings (nursing homes, independent-living residences, regional resource centres). Founded by healthcare professionals, Dr Elise Cabanes and Mariana Fernandes, nurse anaesthetist, EPOCA combines connected devices chosen for each person's situation, the epoca.care platform and a care team providing 24/7 monitoring, in coordination with the patient's GP and geriatrician. Who it is for: people aged 75 and over with at least three chronic conditions who have been hospitalised or have had a fall in the past 12 months. Why: detect early signs of decompensation, act faster and prevent avoidable hospitalisations. Telecare responds to an event that has already happened; medical remote monitoring is clinical follow-up that aims to prevent it. What the data show: in a study published in 2025 in the Journal of Medical Internet Research (80 patients, mean age 86.7, followed by their GP), emergency department visits fell by 62% and unplanned hospitalisations by 57% (retrospective before-and-after study). The solution was evaluated within the French Article 51 Vigie-Âge programme, and the EPOCARE randomised trial is under way in more than 10 hospitals.
