We provide high quality care & support within a safe, friendly & supportive environment, ensuring we manage complex individual needs & improve the quality of life, for anyone over the age of 18, who, because of a learning disability, requires help & support with daily living; whilst two of our homes offer special support to younger adults in transition from children’s to adult services. Each Service User is provided with a Person-Centred Care & Support Plan, which provides the basis for the care & support services they’ll receive, as well as details of their preferred daily routine, their likes & dislikes & any specific dietary requirements or similar needs. The Support Plan also contains risk assessments, risk management plans and details of social interests and activities and how these will be catered for, together with any arrangements to attend religious services. As part of the Community Care Solutions service, our experienced and professional key workers support and encourage residents to enjoy a lifestyle which is as close to normal as possible, whilst promoting independence and encouraging people to make their own choices and determine their own daily routines.
