HOW WE DO IT:
For over 60 years, we’ve provided neuro rehabilitation, behavioral healthcare, developmental support services, autism services, community support, and more. We do this because we believe every person was created to live life to the fullest – no matter their age, no matter their background, no matter what’s holding them back.
WHY IT MATTERS:
60 million people in the U.S. suffer from disabilities or disadvantages that impact how they live. Over one million of these people live in Michigan. To put it into perspective, that’s more than the population of Detroit.
A majority have fallen through the cracks and aren’t receiving the care they deserve. That’s a problem – but we’re solving it by being advocates for these people.

Pathways is a Bay Area not-for-profit organization providing hospice, home health, and palliative care services. Founded in 1977, Pathways serves more than 5,000 families annually in five Bay Area counties.
We care for patients in their place of residence whether it be their home, hospital, nursing home or assisted living facility. Affiliated with El Camino Hospital and Sequoia Hospital since 1986, Pathways Home Health & Hospice is accredited by ACHC. It is a member of the Visiting Nurses Association of America, California Hospice & Palliative Care Association, and the National Hospice & Palliative Care Organization.
