Every industry that manages life and death runs on the same two safeguards: a control tower that plans, clears and validates each action, and an independent, tamper-evident black box that records what was done and what was not. Healthcare built neither. The electronic health record is where clinicians work, but nothing clears or validates an action across the seams a patient crosses, between departments, facilities, shifts, and organizations, and no independent record holds the thread as responsibility changes hands. The harm arrives one patient at a time, so the system was never forced to see it as a single, systemic failure. CorePatient exists to close that gap. We are naming and building the category healthcare never had: clinical action governance, the safeguard every other high-stakes industry adopted only after it could no longer look away.
