Connected Mind is a clinically validated behavioral health screening and testing platform built for primary care. One screen covers six conditions — depression, anxiety, ADHD, bipolar disorder, substance use disorder, and somatic symptom disorder — in under a minute. When the screening flags a condition, validated deeper testing starts automatically. A clinical report is delivered to the chart before the provider reaches the exam room. Every screening is billable under CPT 96127. Deeper testing bills under CPT 96130, 96136, and 96138. Billing guidance is included in every report.
