Community Health Partnership (CHP) was formed in 1992 by health care leaders to foster a coordinated approach to health care issue in El Paso County. Our role is to facilitate collaboration among health care providers and to identify ways to work togethe rto maximize available community resources. We identify and reduce gaps and duplication of services; improve communication among agencies delivering health care serivces, and identfy barriers that negatively impact those seeking health care.
