Community Care Collaborative Information

The Community Care Collaborative (CCC) is a public/private partnership between Central Health and Seton Healthcare Family designed to better manage the care of the shared safety-net population. • The CCC works closely with the Dell Medical School at The University of Texas at Austin, Seton Healthcare Family, Austin Travis County Integral Care and local federally qualified health centers. • The goal of the CCC is to develop an integrated delivery system (IDS) for patient care in Travis County • The IDS will offer coordinated health care services across a spectrum of primary care, specialty, hospital and social service providers • The IDS is designed to improve health outcomes, and serve as a model for high-quality, cost-effective, and person-centered care
