We aim to assist individuals and their family and carers manage these medical conditions, through multidisciplinary medical and psychosocial interventions offered at our clinic in North Caulfield and in your home. Our clinicians are specialists in the assessment, monitoring and treatment of cognitive deficits, neurological and cardiovascular disease. Utilising research validated strategies, we provide education, counselling and support, identify and treat reversible risk factors, facilitate positive lifestyle change, and provide strategy training to maximise independent living skills and quality of life, minimise carer burden, and liaise with referring GPs and physicians. Our clinicians can provide private hospital inpatient care, and when required assist facilitate transitional care. Older adults commonly experience difficulties with the onset or progression of multiple medical conditions. This may impact on their physical and mental capacity to maintain employment, driving, participation in usual hobbies, interests, physical activities and social engagement. Declining vision, hearing, mental acuity, mobility and balance, bladder and bowel function, sleep disturbance, fatigue, anxiety and depression, and physical frailty may lead to loss of independent living skills and deteriorating quality of life. Life transitions associated with physical and/or cognitive decline are often challenging for both the individual and their family and friends. Accurate and timely multidisciplinary assessment, diagnosis, prevention and treatment of modifiable disease risk factors have been shown in clinical studies to improve functional capacity, quality of life and reduce carer stress.
